We are making some changes to our nursing appointments to provide a more streamlined service for your annual reviews.

You may now be asked to book two appointments. The first will be with our Healthcare Assistant, Kyley, who will carry out blood tests, blood pressure checks, BMI measurements, and ask some diet and lifestyle questions.

Your second appointment will be with a member of our nursing team, Bethany or Amber. During this follow-up, they will review your results with you and carry out a more detailed assessment of your long-term conditions.

Patients who are diabetic or on the chronic kidney disease register will be asked to provide a urine sample at their first appointment. If possible, please collect a sample pot before your appointment and bring it with you. If you are unable to do this, you will have the opportunity to return a sample afterwards.
